Purchase & Participation Agreement

By purchasing a ticket and/or attending a Hypnotic Sound Bath session, you acknowledge that you have read, understood, and agreed to all the terms outlined in this Release & Acknowledgment in full. Your purchase constitutes your electronic signature and full acceptance of these terms. If you do not agree with any part of this agreement, please do not purchase a ticket or attend the session.

Liability

I, (the Participant), hereby release LifeSpark Hypnotherapy LLC, Danielle Antonson, Sacred Healing Massage and Bodywork, and Kaitlyn Halwani (collectively, "the Practitioners") from any liability or claims that could be made against them concerning my mental, emotional, and/or physical well-being arising from my participation in the Hypnotic Sound Bath session(s) outlined and agreed upon by signing this form.

Scope of Practice

I understand that Danielle Antonson and Kaitlyn Halwani are not licensed physicians, psychologists, or medical practitioners of any kind, and that hypnosis and sound healing should not be considered a replacement for the advice and/or services of a psychiatrist, psychologist, psychotherapist, or doctor. I understand that the Hypnotic Sound Bath sessions are intended for relaxation and general well-being purposes only.

Health Disclosure

I confirm that I have considered my own health and physical condition before choosing to participate. I agree to inform the Practitioners before the session if I have any of the following: a history of seizure or epileptic disorder, severe mental health conditions including psychosis or dissociative disorders, recent head trauma, active pregnancy concerns, hearing sensitivity or recent ear surgery, a pacemaker or implanted medical device, or any other condition that may affect my ability to safely lie still on the floor for 45–60 minutes or be exposed to continuous sound vibration. I understand that I participate at my own risk and that I am responsible for disclosing relevant health information.

Participation

I give Danielle Antonson and Kaitlyn Halwani full permission to lead me through a Hypnotic Sound Bath session, which may include guided hypnotic suggestion, continuous sound from singing bowls and other instruments, and periods of stillness and deep relaxation. I understand that I play an important role in my own experience by participating fully and openly, and that I am free to leave the session at any time if I become uncomfortable.

No Guarantee of Results I understand that although hypnotic sound bath work has many reported benefits, the Practitioners cannot and do not guarantee specific results, since my personal experience depends on many factors that the Practitioners have no control over, including my own openness and physical/emotional state.

Emotional Release Acknowledgment

I understand that hypnosis and sound healing can occasionally surface unexpected emotional content, including but not limited to memories, sensations, tears, or temporary emotional discomfort. I acknowledge that this is a normal part of the work and not a sign of harm. I agree to seek appropriate follow-up support from a licensed professional if I feel I need additional care after a session.

Physical Touch

I understand that Hypnotic Sound Bath sessions are conducted without physical contact between the Practitioners and Participants. The Practitioners will guide the session through voice and sound only. If Physical touch is necessary, the Practitioners will ask permission. 

Audio & Photo Recording

I understand that during Hypnotic Sound Bath sessions, the Practitioners may occasionally take photos or audio/video recordings of the space for marketing, educational, or archival purposes. The Practitioners will use reasonable efforts to avoid identifying participants in any published materials. I understand that if I do not wish to appear in any photo or recording, I will inform the Practitioners before the session begins. By signing this form, I grant permission for the Practitioners to use such materials, with my identity kept private unless I have given separate written permission.

Property & Personal Belongings

I acknowledge that I am responsible for my own personal belongings during the session. The Practitioners and Sacred Healing Massage and Bodywork are not responsible for any items lost, damaged, or stolen during my time on the property.

Premises Liability

I acknowledge that the session is held on third-party property at Sacred Healing Massage and Bodywork. I agree to enter and exit the space carefully, follow any safety instructions provided by the Practitioners, and assume the risks ordinarily associated with floor-based group activities, including but not limited to lying down, rising, and moving in a low-light environment.

Confidentiality

I consent that the Practitioners may release information to a specific individual or agency if it has been determined that a child or elder is at risk of or is currently being abused; if I, as a participant, am in imminent danger to myself or others; or if a subpoena of records has been requested. I also understand that the Practitioners may discuss aspects of my participation with professional colleagues, keeping my full name and identity completely confidential unless I have given permission otherwise.

Group Confidentiality

I understand that Hypnotic Sound Bath sessions are held in a small group setting. I agree to respect the privacy of other participants and not share their identities, statements, or experiences outside the session.

Acknowledgment

By purchasing a ticket to a Hypnotic Sound Bath session, I acknowledge that I have read this form in full, understand the nature of the sessions, the risks involved, and my responsibilities as a participant. My purchase constitutes my electronic signature and my full and voluntary agreement to these terms
